External deadlines help. Schedule the deadline, and when you will start. Go into a quiet room and psych up for the task. You know it's distasteful and will cause anxiety, but when you finish, reward yourself. Don't plan the reward. Surprise yourself with what you want at the time. I can't say there is a cure for fear related procrastination for we who push ourselves, but we at least have "breathers" in between, if we plow through and then reward ourselves.
